Croydon Healthy Homes

Croydon Healthy Homes supports Croydon households struggling to heat their homes and pay their fuel bills.

The scheme is for people who own their home or rent it from a private sector landlord, and is aimed at families at risk of being in fuel poverty or with a member living with a medical condition which could be exacerbated by living in a cold home.

Qualified Energy Assessors can visit residents and support them to increase the energy efficiency and thermal comfort of the homes and reduce fuel poverty by:

  • Ensuring all timers and controls are set appropriately
  • Offering advice about reducing their energy use
  • Fitting FREE small energy saving measures,
  • Helping residents get onto the best fuel tariffs for them

There is no charge for a Home Visit or any of the measures fitted (Home Visits will be allocated on a first come, first served basis).

Referrals (or self-referrals) for a Home Visit Using take place online: www.croydon.gov.uk/healthyhomes; call freephone number 0800 292 2529 (Monday to Friday 9am to 5pm) or by completing a paper referral.

Do you know?

We have an in-house social prescribing link worker

Saera, our Linkworker, will empower you to take control of your health and wellbeing.  She is non-medical but will help to connect you to community groups and services for practical and emotional support eg debt advice, helping you to fill in taxi forms etc.

Social prescribing can help to strengthen community resilience and personal resilience, and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities. It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

Saera is here twice a week: please speak to your GP or Reception to get an allocated time slot.

We offer in-house minor surgery

WE OFFER IN-HOUSE MINOR SURGERY
We are proud to be an NHS accredited service and offer minor surgery procedures to our patients (as well as accepting referrals from other Croydon practices via your GP).
For referral please see a GP at the practice.

Services include:

  • Injections
  • Aspirations
  • Incisions
  • Excisions
  • Skin biopsies

We offer long term contraception to our patients

Please book an appointment at Reception to discuss long term contraception eg coils or implants which we offer at the practice.

We have achieved silver investors in people accreditation

Getting silver means that not only do we have the right principles in place to lead, support and improve our staff, but that we make active efforts to check they’re happening consistently.

Thank you to all the staff who gave feedback to the independent assessor.

We offer group consultations

Our patients tell us they want to spend more time with the clinician and to be linked up with people living with the same or similar health conditions as themselves. In the Group Consultation you receive the personal attention of your practice team in a calm, relaxed, well organised group setting. Imagine you get not just an unhurried appointment, but in fact a whole 60-90 minutes with your healthcare team, with 45 minutes of this spent with the GP.

Currently Booking:

  • Health Lifestyle Group Consultations
  • COPD Group Consultations

Please ask for the Administration Team who will add your name and provide you with the dates and further information.

You can access feedback from our patient’s every month

Click on the patient feedback page on the front page menu to read a selection of comments posted by our patients monthly and how we have scored on the national Friends & Family Test.

We are a training practice

We are proud to be an accredited practice working with our local hospital trusts in training the GP’s of the future.

Group Consultations

One Thornton Heath Primary Care Network are offering Video Group Consultations to provide support to people with Type 2 diabetes or Hypertension.

The session is a 90 minute virtual group (on Microsoft Teams) with 4-8 patients, a facilitator and a doctor.

What are patients saying?

“I found the session very useful. The most useful part was hearing others peoples experiences and problems and seeing my test results in perspective against the group”

“I found this video call so much better than a 10 minute consultation with a GP as other participants asked questions I may not have thought to ask… the video call more than exceeded my expectations”

“90% of people would recommend the session to family and friends.”

What happens in a Video Group Consultation?

– Discuss your test results
– Answer your questions
– Provide important advice on the management of your condition

What are the benefits of video group consultation?

– Video group consultations give you more options for the support you need.

– People find that video group consultations give them more time for questions and discussion with the clinician.

– They also provide the chance to learn from other people’s experiences.

If you are interested in joining a session or for more information please contact Luciana Castorina on vgc.1th@nhs.net or call 07704 560397.
